Important Information

By registering your son/daughter/ward to the Arizona Pro Soccer Camp sessions you agree to the following CONSENT and WAIVER:
Parental Consent and Waiver:
As the parent/guardian, I certify that my son/daughter/ward has my permission to participate in this program. There is inherent risk in the performance of soccer skills and in the competition that exists between camp participants. All medical bills, regardless of their nature, whether for sickness or injury shall be the responsibility of the camper, and not Arizona Pro Soccer Camp or Salpointe Catholic High School. I understand that should a health emergency arise, I will be notified, but if I can’t be reached such medical treatment as deemed necessary by competent personnel is authorized.
Arizona Pro Soccer Camp will have a certified Athletic Trainer on staff.
As the parent/guardian, I also agree to assume all risks and hazards associated with my child’s participation. To the fullest extent permitted by law, I agree to waive, release, and discharge any and all claims against Arizona Pro Soccer Camp and Salpointe Catholic High School.
Program staff will provide reasonable supervision and follow CDC recommended practices to the extend feasible to minimize risk of exposure to Covid 19 or heat related illnesses and injuries.
Please do not send your child to our session when he/she have any Covid related symptoms.

• Bring water and/or a sports drink.
• Wear a hat and use sunscreen.

Any balance due  can be paid the first day of camp.  If you requested a soccer ball or would like to purchase one, they will be distributed at the camp. ARRIVE EARLY for first day check-in. 

• Bring your soccer ball.
• Bring water and/or a sports drink.
• Bring soccer shoes for the field sessions.
• Bring tennis shoes for the indoor session.
• Temperatures may reach 100°F.
• Wear a hat and use sunscreen.
• Bring a snack for the break period.
